Key Responsibilities- Vital Signs Monitoring: Accurately measures and records patient vital signs as directed by the Registered Nurse (RN) or Licensed Vocational Nurse (LVN).
- Patient Rounds: Conducts patient rounds as instructed; promptly informs the primary nurse if unable to complete tasks.
- Reporting: Monitors vital sign ranges and reports any irregularities to the primary nurse without delay.
- Intake and Output Management: Ensures accurate measurement and documentation of fluid and food intake/output in patient records during each shift.
- Personal Care: Provides bathing and grooming services as scheduled, notifying the nurse and program director if the patient declines assistance.
- Oral and Hair Care: Performs oral hygiene and hair care at least once per shift, documenting these actions in the patient's chart.
- Patient Mobility: Assists in turning patients every two hours or as ordered, documenting any refusals or issues in the patient's chart.
- Feeding Assistance: Aids patients with meals as required.
- Transfers: Supports patients during transfers as necessary.
- Bowel/Bladder Assistance: Aids patients with their bowel and bladder needs.
- Call Light Response: Responds to patient call lights promptly, reporting any concerns or complaints to the program director.
- Condition Monitoring: Observes and reports any changes in the patient's condition, alertness, or skin integrity to the primary care nurse immediately.
- Emergency Preparedness: Familiarizes with facility emergency codes and understands the CNA's role in emergency situations.
- Room Maintenance: Keeps patient beds clean and organized, ensuring that call bells and personal items are within reach.
- Infection Control: Practices proper isolation techniques to maintain a safe environment and minimize infection risks.
- Equipment Hygiene: Maintains cleanliness of instruments and equipment according to facility policies.
- Room Cleanliness: Ensures patient rooms are tidy by removing trash and preventing clutter.
- Supply Management: Assists in restocking necessary supplies and equipment, including personal protective gear.
- Professional Conduct: Treats patients, families, and staff with respect and courtesy at all times.
- Shift Communication: Effectively exchanges patient care information during shift changes.
- Documentation: Completes all required documentation accurately and in a timely manner before the end of the shift.
- Supervisory Collaboration: Accepts guidance from supervisors and management positively and professionally.
- Linen Management: Ensures soiled linen hampers are covered, cleaned, and emptied as needed.
- Patient Engagement: Interacts with patients and their families in a compassionate and caring manner.
- Additional Duties: Performs other responsibilities as assigned.
