The Primary Care Physician (PCP) role encompasses the following key responsibilities:
- Accountability for Patient Outcomes: The PCP is responsible for delivering high-quality clinical care and ensuring cost-effectiveness for a panel of up to 450 patients. Building trust and fostering positive relationships with patients is essential for influencing their health outcomes.
- Health Coaching: The PCP acts as a health coach, assisting patients in setting and achieving health goals while providing ongoing support and follow-up.
- Simplifying Care: The PCP prioritizes and simplifies care processes to facilitate actionable behavior changes for patients and their care teams.
Our organization is committed to value-based care, regularly measuring key performance metrics to support PCPs in achieving success and partnership status. These metrics include patient admissions, appointment utilization, patient experience scores, clinical gap closures, and medical cost measures. PCPs are expected to work collaboratively with center and market leadership to meet these goals while fostering a culture of accountability and compassion.
The PCP will work both independently and in collaboration with other healthcare professionals, ensuring compliance with clinical guidelines and participating in quality management initiatives. The role requires a commitment to high standards of patient care and adherence to regulatory compliance.
Essential Responsibilities:
- Provide comprehensive care for patients with acute and chronic conditions, particularly in the geriatric population.
- Take full responsibility for patient care and outcomes, seeking specialist consultations as necessary while remaining involved in the patient's ongoing care.
- Engage with hospitalists for hospitalized patients, regardless of their affiliation.
- Conduct assessments, diagnoses, treatments, and care coordination for patients with complex health needs.
- Lead a care team that includes medical assistants and care coordinators to optimize patient care.
- Collaborate with transitional care teams for patients unable to visit the office.
- Participate actively in center management and assist in patient recruitment efforts.
- Perform additional duties as assigned by management.
Qualifications:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
- Active medical licensure in the state of practice.
- Minimum of one year of clinical experience in a geriatric or family practice setting preferred.
- Board certification in relevant specialties preferred; board eligibility required.
- Basic Life Support (BLS) certification is mandatory.
- Current DEA registration for controlled substances is required.
