Job Title: Hospice RNJob Summary:

Interim HealthCare of West Michigan is seeking a Registered Nurse to join our Hospice division. As a Hospice RN, you will provide high-quality patient care and support to individuals and their families in a compassionate and professional manner.

Responsibilities:
  • Conduct ongoing assessments of patients' physical and psycho-social status
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families
  • Provide education and interventions to support effective pain and symptom management
  • Ensure availability of necessary equipment, supplies, and services
  • Document patient services provided and attend IDG meetings
Requirements:
  • Active, unrestricted RN license in the State of Michigan
  • At least one year of relevant experience as an RN
  • Preferred: one year of management experience, one year of home health experience, and current CPR certification
