Nurse Practitioner

We are seeking a skilled and compassionate Registered Nurse to join our team at The Grand Rehabilitation & Nursing. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents.

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans to meet the unique needs of each resident.
• Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
• Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.

• Current RN licensure in New York State.
• 1+ year of experience in a rehabilitation or skilled nursing setting.
• Strong clinical skills and ability to work independently.
• Excellent communication and interpersonal skills.
• Ability to work flexible hours, including evenings and weekends.
