Position Overview:
We are seeking a dedicated Occupational Therapist to become a valuable member of our team at Concord Post Acute.

Compensation: $60 to $68 per hour

Key Responsibilities:
  • Design and implement effective treatment plans, securing necessary approvals from referring physicians.
  • Administer treatments in accordance with the prescribed physician's plan.
  • Collaborate with the nursing team to train Restorative Aides.
  • Oversee Occupational Therapy Assistants in delivering patient care and related activities, adhering to state regulations.
  • Maintain open communication with supervisors and other healthcare professionals regarding patient progress, challenges, and strategies.
  • Engage in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as required.
  • Contribute to in-service training programs for facility staff.

Document evaluations, daily treatment notes, weekly progress reports, and recertification paperwork in compliance with Occupational Therapy Board regulations and payer requirements.

Record any modifications to treatment as per established policies and procedures. Educate patients' families or nursing staff on maintenance programs and caregiver training in preparation for discharge from therapy services. Participate actively in discharge planning.

Equipment Recommendations:
Advise on and facilitate the procurement of essential durable medical equipment to promote patient independence in mobility, such as braces, walkers, canes, elevated toilet seats, and grab bars.

Ensure any issues with departmental equipment are reported promptly to maintain optimal functionality.


Licensure Requirements:
Must possess a valid Occupational Therapy License.
