Nurse Practitioner Specialty, Nephrology - Medical Office, CA
Work Site Locations:
2444 W. Beverly Blvd., Montebello, CA And 10000 Lakewood Blvd., Downey CA
* Will see patients at the two listed Carelon Health Medical Office locations and other offices as needed.
Work Shift:
5 Days a Week, Monday – Friday: 8:00 am - 5:00 pm
The Nurse Practitioner Specialty, Nephrology is responsible for providing for the health care needs of patients in collaboration with a physician, through advanced assessment and identification of health states including abnormal conditions, diagnosing, treatment therapies and/or evaluation. Performs care in the specialty area of nephrology and patients on dialysis.
Key Responsibilities:
- Utilizes evidence-based clinical practice standards to provide direct clinical services for patients within prescribed guidelines and physician instructions.
- Acts independently to perform physical assessments, diagnostic functions, specialized risk assessments, and make critical decisions regarding a wide variety of health situations, including high risk and crisis situations.
- Acquires patient health histories and evaluates patient, performing physical examinations.
- Orders, interprets and evaluates diagnostic tests to identify and assess patients clinical problems and health care needs.
- Discusses case with physician.
- Formulates and documents care plan.
- Prescribes medication, therapies and/or other forms of treatment.
- Evaluates the patient's response to therapy.
Requirements:
- Requires an MS in Nursing and minimum of 2 years of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
- Current, unrestricted RN license and NP license in the state of California is required.
- Certification in a specialty or minimum of 1 year experience in the specialty area of Nephrology.
- For Paragon and Carelon Health business unit, satisfactory completion of a Tuberculosis test is a requirement for this position and Hepatitis B vaccine or signed waiver.
Preferred Skills, Capabilities and Experiences:
- ANCC and/or AANP Board Certifications highly preferred.
- Bilingual communication skills in English and Spanish.
- Experience in either Primary Care or an Internal Medicine setting with adults and geriatric patient population.
