Job Title: Personal Care Assistant

We are seeking a compassionate and skilled Personal Care Assistant to join our team at Trinity Health PACE. As a Personal Care Assistant, you will play a vital role in providing essential personal care services and health support to our participants.

Job Summary:

This is a full-time position that requires a high level of independence, adaptability, and strong communication skills. You will work closely with our interdisciplinary team to provide comprehensive services, allowing our participants to remain independent at home.

Responsibilities:
  • Conduct ongoing participant assessments and monitoring to identify safety hazards and respond to emergencies.
  • Provide assistance with daily activities, including toileting, grooming, and meals.
  • Aid in maintaining physical activity and exercise routines.
  • Escort participants to appointments and assist with transportation.
  • Report any changes in participant's condition to center staff.
Requirements:
  • High School Diploma or equivalent.
  • Minimum of one (1) year of documented experience working with a frail/elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.
  • Must be willing to learn American Sign Language (ASL). ASL proficiency preferred.
  • Valid driver's license, insurance, and use of personal vehicle.
