Mental Health Case Manager-Co-Therapist

About the Role

We are seeking a highly skilled Mental Health Case Manager-Co-Therapist to join our team at the Community Counseling Center of Mercer County. This role is responsible for providing direct treatment services to children, adolescents, and families using the Ecosystemic Structural Family Therapy model and other treatment modalities.


Key Responsibilities
  • Conduct assessments and provide treatment services in the home of the identified client/family.
  • Provide crisis intervention as needed.
  • Offer supportive counseling, psychotherapy, and skill training in areas such as child management, problem-solving, and stress reduction techniques.
  • Develop crisis-safety and treatment plans, provide case management and linkage, and plan discharge for assigned cases.
  • Maintain intensity of service as many hours as needed per week with each enrolled family.
  • Ensure child's safety and well-being by monitoring home environment and facilitating placement if necessary.
  • Attend individual, team, and group supervision meetings for clinical guidance and chart reviews.
  • Maintain consumer files in compliance with licensing requirements.
  • Participate in on-call rotation and work a flexible schedule including evenings and weekends based on family needs.

Requirements
  • Bachelor's degree in psychology, social work, nursing, education, rehabilitation, pre-med, theology, or anthropology, plus 1 year of experience in a CASSP system program.
  • 12 college-level semester hours in humanities or social services, plus 1 year of experience in a CASSP system program, and enrollment for certification by OMH as a mental health family-based worker.
  • Licensed RN, with 1 year of experience in a CASSP system program, and enrollment for certification by OMH as a mental health family-based worker.

Estimated Salary Range

$45,000 - $65,000 per year, plus shift differential for hours worked after 6pm.
