We recognize that skin is a vital indicator of our patients' health and the overall health of our nursing centers. Therefore, we are in search of compassionate and driven Registered Nurses - RNs with a focus on wound care to lead and integrate a patient-centered approach for skin health and wound management as a Wound Care Coordinator - RN.

Position Highlights:
- Lead and implement a comprehensive Skin Health and Wound management program in collaboration with nursing center management.
- Serve as the clinical expert for skin, wound, and incontinence products within central supply.
- Oversee durable medical equipment related to wound prevention and management.

Qualifications:
- Must possess a valid RN license (MPT/DPT with leadership review/approval)
- Completion of Skin Health Team Lead orientation training and Wound Basics within 90 days of hire is required. Commitment to attend monthly support calls is mandatory.
- Must have a nationally accredited wound certification (CWS, CWCN, WCC, CWON, WOC, AWCC) or be committed to completing Advanced Wound Training and national accredited Wound Certification within one year of hire.
- Knowledge and experience with clinical charting, incident reporting, and investigation response are essential.
- Strong understanding of acceptable standards of care for nurses, providers, therapists, dietitians, and other team members.
- Experience in team building and respect for the interprofessional team in wound management is required.
- Familiarity with national wound guidelines and wound products is necessary.
- Knowledge of nursing home regulations and the survey process is required.
- Proficiency in Microsoft Word, Excel, Database Management, and PowerPoint is essential.
