About the Role:
  • Coordinate the transfer and placement of patients across Memorial Healthcare System facilities
  • Communicate effectively with surgery department, emergency department, nursing units, physician offices, and outlying facilities to ensure timely and efficient transfer and placement
  • Provide high-priority customer service and ensure complete customer satisfaction
Responsibilities:
  • Remotely engage with facility staff to monitor and document transfer needs
  • Communicate patient condition to sending or receiving providers and facilities
  • Consult with other disciplines to determine patient needs, such as infection control, equipment needs, nutritional, functional, pain, social, and skin integrity
  • Serve as an expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervene as needed
  • Follow established protocols for interfacility transfer acceptance and escalation to leadership
  • Document transfer and bed placement in the Epic EMR to ensure adequate patient safety standards
  • Collaborate across disciplines to coordinate patient transfers, including disposition, ambulance transportation, specialist referrals, and bed assignments
  • Anticipate admission needs and communicate findings with transferring departments and facilities
  • Plan, implement, and evaluate patient needs to optimize outcomes and maximize available resources
Requirements:
  • Bachelor's degree in Nursing (Required)
  • Minimum of three (3) years of Emergency Department or Critical Care experience
  • Current Registered Nurse License (RN) in the State of Florida
  • BLS Certification from the American Heart Association
