Job Title: Registered Nurse Case Manager

Job Summary:

The Registered Nurse Case Manager is responsible for coordinating patient care and promoting quality outcomes through effective communication and collaboration with the multidisciplinary healthcare team. This role requires strong analytical and critical thinking skills, as well as the ability to navigate complex healthcare systems and advocate for patients' needs.

Key Responsibilities:

  • Provide patient-centered care by coordinating the plan of care, managing length of stay, and ensuring appropriate resource utilization.
  • Collaborate with the multidisciplinary team to develop and implement a comprehensive discharge plan that meets the patient's needs and preferences.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Identify and address potential barriers to care, including social determinants of health, and develop strategies to mitigate these risks.
  • Participate in performance improvement activities to identify and address opportunities for improvement in patient care and outcomes.

Requirements:

  • Associate Degree in Nursing or Nursing Diploma Required
  • Bachelor's Degree in Nursing Preferred
  • 2+ years experience in case management or 3+ years experience in clinical nursing Required
  • InterQual experience Preferred
  • Certification in Case Management Preferred
  • Ability to establish and maintain collaborative and effective working relationships
  • Ability to communicate effectively in oral, written, and electronic formats
  • Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills
