Key Responsibilities- Conduct comprehensive patient assessments and develop individualized care plans to meet patients' unique needs.
- Provide skilled nursing care, including wound care, medication management, and patient education, to patients in their homes.
- Collaborate with our clinical team, including physicians, therapists, and caregivers, to ensure seamless care coordination and communication.
- Document patient care activities and outcomes in our electronic medical record system.
- Participate in quality improvement initiatives to enhance patient care and satisfaction.
- Current and unrestricted California Registered Nurse license.
- Minimum of 1 year RN experience in an institutional setting, such as acute care, rehabilitation, or home health.
- Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is highly desirable.
- Ability to chart with Electronic Medical Record Software (Wellsky or Kinnser a plus).
