Job Title: Registered Nurse - Home Care

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Care Registered Nurse. As a key member of our healthcare team, you will provide high-quality patient care and support to individuals in their homes.

Job Summary:

The Registered Nurse (RN) is responsible for delivering competent, quality patient care via the development, implementation, and evaluation of individual patient care plans. You will work closely with patients, families, and healthcare teams to ensure seamless care coordination and excellent patient outcomes.

Responsibilities:
  • Develop, implement, and evaluate patient care plans that meet the unique needs of each individual.
  • Provide direct patient care, including observation, assessment, nursing diagnoses, counseling, care, and health teaching.
  • Maintain ongoing responsibility for assigned caseload and coordinate associated services.
  • Evaluate admissions to ensure they meet established criteria.
  • Develop, implement, and evaluate individual patient plans of care.
  • Maintain communication with supervisors and record review committees as needed.
  • Function as a liaison between the Agency and patients and their families to interpret and explain services.
  • Provide teaching and supervision to patients and families on providing care at home.
  • Schedule workload for maximum efficiency.
  • Participate in Agency evaluation programs as instructed.
  • Regularly confer with other healthcare team members to maintain effective coordination of patient care.
  • Abide by and promote Agency administrative policies.
  • Maintain a continuing knowledge of competencies related to the nursing professional through participation in formal education programs, conferences, workshops, and professional organizations.
  • Regularly consult with patients' physicians regarding plans of care and patient status in response to prescribed care.
  • Participate in staff in-service programs and supervise and teach other nursing personnel, including home health aides.
  • Write discharge summaries upon discontinuance of service/plan of treatment to include general course of care, condition on discharge, and to whom discharged.
Requirements:
  • Minimum of six (6) months recent work experience as a home care nurse or have a public health nursing certificate per state requirement or six (6) months home health or community health nursing or two to three (2-3) years acute hospital nursing.
  • Current Nursing licensure in your state of employment.
  • Graduate of an approved School of Nursing.
  • Satisfactory references.
  • Satisfactory completion of Agency RN Qualifying Exam.
  • Current CPR certification.
