Assess training requirements and growth opportunities for team members.
Address performance-related issues with staff to uncover underlying factors and collaborate with Human Resources to resolve challenges.
Engage in the duties of team members to enhance efficiency or assist in overcoming challenging tasks.
Review and appraise the job performance of team members, recommending suitable personnel actions as necessary.
Conduct in-home and telehealth assessments of patients, evaluating their health status, strengths, care requirements, and preferences using the UAS-NY assessment tool.
Support and mentor new Nurse Evaluators to ensure they fulfill the essential responsibilities of their role.
Facilitate CFEEC appointment visits to provide training and education for new Nurse Evaluators, demonstrating assessment techniques and offering constructive feedback to the Regional Nurse Manager. Communicate any concerns to the Regional Nurse Manager.
Assist in the training of new Nurse Evaluators and provide refresher training for existing staff as required.
Identify performance gaps among staff and implement corrective measures to address deficiencies.
Collaborate with the Regional Nurse Manager to offer assessment guidance to team members.
Essential Qualifications:
Active RN Licensure in New York State
- Minimum of 2 years of RN experience in home care, including at least 1 year of experience utilizing the UAS-NY assessment tool
- Proficient in reviewing health status inquiries and making evaluations based on UAS criteria for enrollment; adept at utilizing technology in a professional setting.
Understanding of the principles, theories, and practices of general professional nursing.
Familiarity with state and agency regulations governing professional nursing practices.
