Job Title: Travel Nurse RN Case Management

We are seeking a skilled Travel Nurse RN Case Management to join our team at Travel Nurses, Inc.

Job Summary

This is a 13-week travel nursing job opportunity for a Case Management RN in Santa Rosa, California. The ideal candidate will have 1-2 years of recent experience in Case Management and be willing to adapt to new environments.

Responsibilities
  • Provide case management services to patients in a hospital setting
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care transitions
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Communicate effectively with patients, families, and healthcare teams to ensure optimal care outcomes
Requirements
  • Current RN licensure in the state of California
  • 1-2 years of recent experience in Case Management
  • Ability to work 40 hours per week, 8 hours per shift, with flexibility to adapt to changing patient needs
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced hospital environment
