About the Role

We are seeking a compassionate and skilled Hospice Medical Social Worker to join our team at Agape Care Group. As a Hospice Medical Social Worker, you will play a vital role in providing emotional support and psychosocial evaluations to patients and their families during their end-of-life journey.

Key Responsibilities
  • Conduct thorough psychosocial evaluations to identify patient needs and develop personalized care plans
  • Provide ongoing counseling and support to patients and families, addressing their emotional, social, and spiritual needs
  • Collaborate with the interdisciplinary care team to ensure comprehensive and coordinated care
  • Document patient progress and outcomes in accordance with regulatory requirements
Requirements
  • Master's degree in Social Work (MSW) from an accredited program
  • At least 2 years of clinical experience in healthcare or hospice care
  • Ability to sit, stand, bend, and move intermittently, with the capacity to lift at least 25 pounds
