General Overview


Under minimal oversight, this role is accountable for managing a caseload and referrals, as well as overseeing the operations of a designated Henry Ford at Home team.

Utilizes nursing principles to guarantee safe and effective coordination of patient care within their residence.

Conducts responsibilities typical of a Home Health Care nurse.


Qualifications & Experience

Graduation from an accredited Registered Nurse program is required. A current license as a registered nurse in the State of Michigan is mandatory. A minimum of one (1) year of progressively responsible experience in home health care nursing is essential. Proficiency in computer usage for electronic documentation of patient care is necessary.

Strong interpersonal and customer service skills are essential for effective interaction with individuals from diverse cultural and socio-economic backgrounds.

A valid State of Michigan driver's license, a satisfactory driving record, and reliable transportation are required. The ability to lift, carry, transport, and assist clients as needed is essential.
