House Calls Nurse Practitioner Opportunity

Optum is seeking a skilled Nurse Practitioner to join our House Calls team in Ouachita, AR. As a member of our integrated medical team, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate follow-up care to improve health and well-being.

Key Responsibilities:
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing as appropriate
  • Identify diagnoses to inform care management and active medical management in the furtherance of treatment
  • Communicate findings to inform members' primary care physicians of potential gaps in care
  • Intervene in urgent and emergent situations as needed
  • Educate members on disease processes, medications, and compliance
Requirements:
  • Active NP license or ability to obtain by start date, with unencumbered licensure
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
  • Reliable transportation for home visit assessments
Preferred Qualifications:
  • 1+ years of clinical experience in the highest level of education, clinical setting
  • Ability to travel throughout the state, up to 5% of the time, with advanced notice and potential for additional compensation incentives
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
