Position Overview
This role as a Registered Nurse Care Manager involves working across various inpatient adult units, including medical/telemetry, orthopedic/spine, and general surgery.

Key Responsibilities
- Float between different units as necessary.
- Engage with multiple community partners to develop care plans and obtain insurance authorizations.
- Assist with durable medical equipment and facilitate home health services, as well as coordinate various post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living.

Job Requirements
- Experience in inpatient adult care settings, specifically in medical/telemetry, orthopedic/spine, and general surgery.
- Flexibility to float between units as required.
- Availability for occasional weekend shifts.
- Ability to manage patient ratios of 1:17.

Schedule Information
- The position may require floating between units.
- On-call duties are not a requirement.
- Weekend work may be necessary on occasion.
- Block scheduling is not applicable for this role.

Special Considerations
- Knowledge of utilization review (UR) is beneficial, although not a primary function of this role.
- This position plays a crucial role in ensuring quality patient care through effective management and coordination.
