Job Title: RN Case Manager - Field Based

Key Responsibilities:
  • Assess, plan, and implement care strategies tailored to individual Medicare and Medicaid members, while proactively engaging them to manage their own health and healthcare.
  • Utilize strong skills in managing multiple tasks, being self-motivated, and driving quality results, with a focus on time management, attention to detail, and teamwork.
  • Work in a high-volume, high-priority customer service environment, providing efficient, productive, and thorough support to members over the phone and in person.
  • Proactively engage members to manage their health and healthcare using Motivational Interviewing Skills, and manage transitions of care for members discharged from in-patient hospital stays and emergency rooms.
  • Support members in improving their wellbeing by staying out of the hospital, attending regular visits to their primary physician and specialists, and identifying and initiating referrals for social service programs.
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members, and advocate for individuals as needed to ensure access to care.
  • Provide clinical support for non-clinical staff, educating them on medications, diagnoses, and disease-specific education, and collaborate with the interdisciplinary team to ensure appropriate services and identify needs.
Requirements:
  • Current, unrestricted RN license in the state of Kansas.
  • 2+ years of experience working in the community as a RN in acute in-patient care, long-term care, home health, hospice, public health, behavioral health, or assisted living.
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook.
  • Ability to travel up to 75% of the time with reliable transportation, a current and non-restricted state of Kansas Driver's License, and State-required insurance.
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI).
Preferred Qualifications:
  • Bachelor's degree in nursing.
  • Background in managing Medicaid and Medicare populations with complex medical and behavioral needs.
  • Experience with arranging community resources and electronic charting.
