Home Health Care Coordinator

Masonicare Home Health & Hospice is seeking a skilled RN Hospice Nurse to work with our team of dedicated healthcare professionals. As a valued member of our team, you will provide end-of-life care to patients in their homes or assisted living facilities, ensuring their comfort, dignity, and well-being.
Responsibilities and Expectations- Assess and develop individualized plans of care that address the unique needs and goals of each patient
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings
- Communicate effectively with patients, families, and healthcare professionals to ensure shared understanding and consistency of care
- Educate and support patients and families on self-care, disease management, and other relevant topics
- Maintain accurate and detailed records of patient interactions, assessments, and interventions
We offer a competitive salary range of $40.00-$52.00 per hour, depending on experience, as well as opportunities for professional growth and development within our organization.
