The Physical Therapist plays a crucial role within the multidisciplinary team at Centrix Health Resources. The primary objective of the home health physical therapist is to assist patients in regaining and maintaining their independence within their own homes. This role is dedicated to restoring functionality, alleviating discomfort, and preventing disabilities through the implementation of medically prescribed physical therapy.
Key Responsibilities:
- Collaborates with physicians to assess the patient's functional capabilities.
- Evaluates the patient's home environment for safety concerns and identifies necessary equipment.
- Identifies functional challenges and formulates a care plan aimed at enhancing the patient's functional abilities, adjusting as needed.
- Conducts various assessments, including range of motion and manual muscle tests, and documents findings to inform treatment plans.
- Completes initial and ongoing assessments along with required documentation in accordance with agency protocols.
- Regularly reviews and updates the patient's program to optimize functional recovery.
- Implements exercise regimens utilizing specialized therapeutic techniques and equipment.
- Designs and executes physical therapy treatment plans to restore function, relieve pain, and prevent disability following medical conditions or injuries.
- Advises on prosthetic and orthotic devices, evaluates their effectiveness, and recommends adjustments as necessary.
- Engages with family members, referring physicians, and other healthcare professionals to ensure coordinated and high-quality patient care.
- Submits comprehensive documentation of evaluations, care plans, clinical notes, and progress reports, including any changes in treatment orders.
- Ensures that all patient visit notes are submitted within the designated timeframe.

Qualifications:
Candidates must have graduated from an accredited Physical Therapy program and hold a valid license in the state of Arizona.
