Job Description

The Vocational Nurse - Patient Care Specialist will work under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The role requires strong clinical assessment and documentation skills, as well as excellent communication and interpersonal skills. Key responsibilities include:

  • Delivering high-quality patient care that meets individual needs
  • Conducting thorough assessments and documenting patient information
  • Developing and implementing individualized care plans
  • Collaborating with healthcare teams to ensure seamless care coordination
Requirements

To be successful in this role, you must have:

  • Graduation from an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies
