Job Description

Purpose of Position:

We are seeking a skilled Community Health Nurse to join our team at NYC Health Hospitals/Community Care. As a key member of our interdisciplinary team, you will provide community-based professional nursing care to patients in group and private homes, as well as those without housing across New York City.

You will diagnose and treat responses to actual or potential health problems using nursing methods and techniques such as case-finding, health teaching, health counseling, and supportive care. Your responsibilities will include assessing, planning, implementing, and evaluating patient plans of care, providing direct skilled nursing care, observing, and instructing patients in their place of residence.

Duties & Responsibilities:

  • Provide individualized skilled nursing care to patients requiring part-time intermittent professional nursing service through the use of the nursing process.
  • Assess patients and develop plans of treatment and care with ongoing evaluation of patient/family needs for other services.
  • Provide professional nursing services as outlined in the patient's plan of treatment and authorized by the patient's physician, and provide emergency treatment as needed.
  • Record physical, social, psychosocial, and environmental needs of patients. Provide ongoing monitoring of patients in the home care setting and escalate any clinical changes to the physician or non-physician provider as needed.
  • Utilize input from patients, caregivers, MDs, and peers to devise a nursing care plan that is feasible within the physical, financial, and emotional resources of the patient/caregiver and Department of Health regulations.
  • Evaluate patients' responses to plans of care as necessary, consistent with physician's orders.
  • Review and revise plans of care as necessary, consistent with physician's orders.
  • Plan activities necessary for effectuating timely discharge/transfer of patients and notify all appropriate parties, including patients' physicians.
  • Authorize and coordinate all services, including nursing, rehab, respiratory therapy, nutritional services, equipment, professional and non-professional staffing, and support services.
  • Collaborate with Accountable Care Managers and other disciplines regarding patients' progress, plans of care, and discharge plans.
  • Demonstrate a professional attitude and ethical conduct in compliance with departmental standards.
  • Involves patients and caregivers actively in goal setting.
  • Establishes and maintains cooperative, effective working relationships with staff, MDs, Case Managers, and other hospital personnel, outside vendors, agencies, and the community at large.
  • Attend weekly to bi-monthly team meetings and participate in case conferences and case load reviews on a weekly to bi-weekly basis.
  • Complete physician's orders in a timely manner when needed.
  • Complete admission paperwork, including physician and nursing POCs, within 48 hours of visit encounter.
  • Review clinical records to ensure compliance with departmental and regulatory standards.
  • Obtain written orders for all verbal orders returned.
  • Write clean and concise progress notes that reflect implementation of the plan of care and patients' responses to that plan of care.
  • Maintain up-to-date patient files with complete records, including care plans and re-certifications, reports of all services rendered, progress notes, and discharge summaries.
  • Be competent in knowledge of patient teaching plans and interventions.
  • Be competent in knowledge of safe administration and teaching medications.
  • Demonstrate safety and accuracy in performing treatments and procedures according to Home Care Department standards of care/practice.
  • Be competent to complete thorough and accurate nursing assessments.
  • Demonstrate competence in skilled nursing interventions.
  • Be competent in knowledge of documentation requirements for Home Care.
  • Participate in case conferences and/or Interdisciplinary Team conferences as needed and as designated by the Clinical Manager.
  • Work with other members of the Interdisciplinary Team (IDT) to develop appropriate interventions to achieve clinical and functional goals.
  • Participate in the agency's Quality Assurance Performance Improvement/PIP as assigned.
  • Supervise and monitor the care given by home health aides and ensure proper documentation.
  • Participate in an interdisciplinary healthcare team in determining and implementing a comprehensive plan of care.
  • Participate in the coordination of other professional and para-professional services in the home and provide supervision as needed.
  • Provide patient/family/significant other/patient-identified representatives with teaching and involves them in assessment activities.
  • Maintain current, complete, and appropriate documentation in the patient care record. May be required to use computer systems in carrying out these functions.
  • Duties include but may not be limited to wound care, including NPWT/VAC, Peg/Trach Care Management, Foley insertion and catheter care management, JP/PleuRx Drain care, administering injections.
  • Provide teaching on disease and medication management.
  • Obtain photographs for consultation by wound specialists.
  • Ability to apply evidence-based practices to the home care setting.
  • Other duties as ordered/prescribed.

Estimated Salary:$65,000 - $80,000 per year, depending on experience.

Requirements:

  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
  • One (1) year of experience as a registered nurse.
  • Ability to travel regularly to patient/client assignments across New York City via public transportation and/or private vehicle.
  • Demonstrated commitment to continued professional development.

Preferred Qualifications:

  • Licensed and currently registered to practice as a professional nurse in New York State.
  • Current certification in Basic Life Support procedures including CPR.
  • Baccalaureate Degree in Nursing from an approved program preferred.
  • Minimum two (2) years of full-time nursing experience.
  • At least 2 years of experience in Homecare, Long-term Care, or Hospital setting.
