Licensed Practical Nurse

At Phoenix Senior Living, we are seeking a skilled Licensed Practical Nurse to join our team. As an LPN, you will play a vital role in ensuring the health and wellbeing of our residents. Your responsibilities will include monitoring resident health, administering medication, and communicating with resident support services and families.
Key Responsibilities:
- Assess resident needs and develop personalized service plans
- Administer medication and provide direct resident care as needed
- Communicate with resident support services and families
- Assist with completing clinical sections of monthly wellness visits and PSP
Requirements:
- Must be a Licensed Practical Nurse or Registered Nurse
- A minimum of one year of experience working with elders or disabled individuals, preferred
- Ability to work weekends, evenings, and flexible hours
