LPN - Home Health, Part Time

Mercy Home Health is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality care to patients in their homes. As a LPN, you will assess, plan, implement, and evaluate patient care, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.
Responsibilities- Provide skilled nursing services to patients in their homes, under the direction and supervision of a registered nurse.
- Assess patient needs and develop individualized care plans to meet those needs.
- Implement and evaluate patient care, ensuring that patients receive high-quality, compassionate care.
- Collaborate with our interdisciplinary team to coordinate patient care and achieve optimal outcomes.
- Document patient care and progress in a timely and accurate manner.
- Current LPN license in the state of Missouri.
- One year of experience in a related field, preferably in home health care.
- BLS CPR Certification through American Heart Association.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Flexibility and adaptability to meet the changing needs of patients and the healthcare system.
