Overview:
This role involves working in partnership with healthcare providers, multidisciplinary teams, individual patients, and their families to enhance positive health outcomes. The Care Manager RN is responsible for a comprehensive care process that includes assessment, planning, implementation, coordination, monitoring, and evaluation of services necessary to meet the health needs of individuals. This position emphasizes effective communication, education, and resource utilization to achieve quality and cost-effective results. The role provides essential support across various settings including utilization management, emergency services, acute care, outpatient, and specialty care.

Key Responsibilities:
  • Utilizes critical thinking skills and the nursing process, grounded in research and evidence-based practices, to address the healthcare needs of clients.
  • Builds collaborative relationships with clients to help them assess their healthcare needs, decision-making processes, lifestyle choices, and resource utilization.
  • Serves as an advocate, educator, and coach for clients, families, and caregivers regarding treatment options, community resources, and psychosocial issues to facilitate goal setting and the development of self-care skills.
  • Enhances communication and coordination among healthcare team members, involving clients in the decision-making process to reduce service fragmentation.
  • Demonstrates critical thinking, sound judgment, and the ability to operate with minimal supervision in a fast-paced environment.
Qualifications:
  • Minimum Required Education: Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years' experience. Preferred: Bachelor's degree in Nursing.
    Required Licensure/Certification: State licensure as a Registered Nurse (RN). Preferred: Basic Life Support (BLS) Healthcare Provider, Certified Case Manager (CCM).
    Minimum Experience: 2 years of nursing experience. Preferred: Acute care and 1 year Case Management experience.
