Home Care LPN Specialist

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our team at LiveWell Partners. As a skilled nursing professional, you will work one-on-one with patients in their home environment, providing education and skilled nursing to promote independence.
Responsibilities
Participate in the planning and coordination of total patient care in conjunction with the RN and physician's plan of care.
Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
Prepare clinical/progress notes and summaries.
Assist patients with activities of daily living while teaching appropriate self-care techniques.
Qualifications
1 year experience as an LPN in a home care, med surg, or ED setting.
Current state license as an LPN.
Proof of current CPR certification and valid Driver's license.
Ability to work a flexible schedule.
Excellent interpersonal skills.
