Job Summary:

We are seeking a skilled Occupational Therapist to join our team at Cornerstone Rehab. As a key member of our clinical staff, you will be responsible for delivering high-quality occupational therapy services to our patients in skilled nursing facilities.

Responsibilities:

  • Provide occupational therapy services to adult and geriatric patients, focusing on their unique needs and abilities.
  • Develop and implement individualized plans of care, ensuring seamless transitions and optimal patient outcomes.
  • Collaborate with our multidisciplinary team to ensure comprehensive care and effective communication.
  • Conduct thorough assessments, progress notes, and discharge summaries to document patient progress and outcomes.

Requirements:

  • Valid State Occupational Therapist License and Credentials
  • Experience in SNF and/or Acute care settings
  • Covid Vaccine required in certain locations
