This role is designed as an outpatient-only opportunity, managing a patient panel of 600 individuals primarily on Medicare and Medicare Advantage plans.
Key Features of 65 Prime Plus by Mercy:
- Emphasis on value-based care, allowing for meaningful interactions with patients and quality-driven incentives.
- Support from a dedicated panel RN for telephonic encounters, triage, and in-basket assistance.
- Flexibility in patient interactions, including in-person, virtual, or telephonic consultations for comprehensive annual visits, transitional care, and urgent needs.
- Exceptional work-life balance, with administrative support focused on minimizing barriers to care.
- Appointment slots ranging from 30 minutes to 1 hour, ensuring ample time for patient engagement.
- Manageable patient panel sizes of 600 or 1,000, with assistance from Advanced Practice Providers (APP).
- No requirements for holiday, weekend, or evening shifts.
- Commitment from administration to reduce physician burnout through a comprehensive wellness program.
