Job Title: Clinical Pharmacist

Stanford Health Care is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing pharmaceutical services to our patients.

Responsibilities:
  • Provide clinical pharmacy services, including medication therapy management and drug information.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Conduct medication use evaluations and provide recommendations for improvement.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Stay up-to-date with the latest developments in pharmacy practice and apply this knowledge to improve patient care.
Requirements:
  • Doctor of Pharmacy degree from an accredited institution.
  • California Pharmacy License.
  • 2+ years of experience in clinical pharmacy practice.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
