Your Role:

We are seeking a skilled, compassionate, and experienced Licensed Practical Nurse (LPN) for a full-time role in the Braintree, Holbrook, and Weymouth regions.

In this position, you will collaborate closely with the RN case manager to deliver skilled nursing care to patients in their residences.

Your responsibilities will include direct patient care and promptly communicating any changes in patient status to the physician and case manager.

Effective teamwork and strong communication skills are essential.


Qualifications:
  • Current Massachusetts Licensed Practical Nurse license in good standing.
  • Minimum of 2 years of experience as an LPN.
  • Prior experience in homecare is preferred.
  • Strong clinical assessment abilities and the capacity to prioritize and triage patients.
  • Exceptional communication skills and the ability to function effectively within a team.
  • Excellent organizational and time management skills.
  • Ability to work independently and knowledge of how to access patient resources.
  • Proficient computer skills.
