Join Our Team as a Home Health LPN

Become a part of our caring community and help us put health first. As a Home Health LPN, you will play a vital role in providing high-quality care to patients in the comfort of their own homes.

Key Responsibilities:
  • Provide professional nursing services under the direction of a RN in compliance with state and federal regulations, applicable healthcare standards, and CenterWell Home Health policies and procedures.
  • Deliver skilled nursing interventions in the treatment of patients' illnesses, rehabilitative needs, and preventative care.
  • Utilize a holistic approach in the provision of problem-specific therapeutic interventions, teaching, and training activities in accordance with the plan of care.
  • Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.
  • Collaborate with the RN Supervisor to ensure seamless care coordination and make necessary changes to the Plan of Treatment.
  • Communicate effectively with patients, physicians, clinical managers, and other healthcare team members regarding patient needs and status changes.
  • Participate in care coordination activities and discharge planning as appropriate.
  • Maintain confidentiality and adhere to professional standards in the provision of care.
Requirements:
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
