Job Summary:

As a Community Care Worker I, you will determine eligibility for Community Care Services Eligibility (CCSE) services, conduct home visits, interview individuals, and develop service plans.

Key Responsibilities:
  • Conduct home visits and interviews to assess the need for services
  • Determine eligibility and functional needs
  • Obtain, verify, and calculate income and resources
  • Maintain case record documents and enter information in an automated system
  • Monitor services to determine if they are meeting the individual's needs
  • Develop and coordinate service plans with individuals and their families
Requirements:
  • Ability to effectively communicate orally and in writing
  • Ability to set priorities, establish timeframes, and meet deadlines
  • Ability to establish and maintain effective relationships with individuals, co-workers, and staff from other agencies
  • Knowledge of interviewing techniques and resources that serve individuals who are older or have a disability
