We are seeking a skilled Healthcare Professional to fill the position of Care Management Specialist. As a key member of our team, you will play a vital role in delivering high-quality care to our members.
Job DescriptionThe Care Management Specialist will be responsible for assessing, developing, implementing, and evaluating care plans to optimize member healthcare across the continuum. This will involve conducting thorough assessments to identify individual needs and developing specific care management plans to address objectives and goals. The successful candidate will also facilitate authorizations/referrals as appropriate within benefits structures or through extra-contractual arrangements, coordinate internal and external resources to meet identified needs, monitor and evaluate the effectiveness of care management plans, and modify as necessary.
RequirementsTo be successful in this role, you will require a BA/BS in a health-related field and a minimum of 5 years of clinical experience, or any combination of education and experience that would provide an equivalent background. You must also hold a current, unrestricted RN license in the applicable state(s) and have multi-state licensure if providing services in multiple states.
Preferred Skills and QualificationsCertification as a Case Manager is preferred, along with compact license, Kentucky RN License, and case management certification and/or experience. For URAC-accredited areas, you will require a BA/BS and a minimum of 5 years of clinical care experience, or any combination of education and experience that would provide an equivalent background, and a current and active RN license in the applicable state(s). Certification as a Case Manager and a BS in a health or human services-related field are preferred.
BenefitsAs a valued member of our team, you can expect a competitive salary range of $85,000-$115,000 per annum, depending on experience, plus a comprehensive benefits package that includes merit increases, paid holidays, paid time off, medical, dental, vision, short and long-term disability benefits, 401(k) + match, stock purchase plan, life insurance, wellness programs, and financial education resources. We operate in a hybrid workforce strategy, requiring associates to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process.
