- Collaborate with healthcare teams to coordinate care and ensure seamless transitions between settings.
- Prescribe medications and perform procedures as needed to address acute and chronic conditions.
- Document patient information accurately and efficiently in electronic medical records.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Master's degree in family health from an accredited nursing program.
- Active and unrestricted national certification from a recognized organization (e.g., American Academy of Nurse Practitioners, American Nurses Credentialing Center).
- Current state-issued license to practice as a Family Nurse Practitioner.
- Basic Life Support for Healthcare Professionals certification.
- Unrestricted driver's license.
- 3-5 years of experience as a Registered Nurse and/or Nurse Practitioner, preferably in home health, community health, or public health settings.
- Previous experience in home health as a licensed clinician, especially in managing chronic conditions.
- Experience working with underserved populations facing socioeconomic barriers to healthcare.
- Fluency in a language in addition to English is a plus.
- Immunization and point-of-care testing skills.
