Job Overview

Position Objective:

Supervisory Responsibilities:

Oversees all staff members on duty during the assigned shift, including housekeeping, home health aides, and volunteers.

Qualifications:

  • High School Diploma or GED equivalent.
  • Graduation from an accredited practical nursing program.

  • A minimum of two years of relevant nursing experience is required.

Key Responsibilities:

  • Delivers direct nursing care to hospice residents and supervises nursing assistants, collaborating closely with staff and volunteers.
  • Actively participates in assessing patient needs and developing care plans.
  • Communicates any changes in patient conditions to the RN Case Manager, Team Leader, or attending physician.
  • Maintains accurate documentation in patient records.
  • Assists with meal preparation for residents as needed.
  • Ensures a clean and safe environment for residents at all times.
  • Supports the House Manager in upholding health, fire, and safety standards in accordance with policies.
  • Adheres to infection control protocols at all times.
  • Assists the Team Leader in evaluating the performance of CNAs and other staff members.
  • Provides mentorship and guidance to team members, ensuring they receive necessary training and support.
  • Engages in the Individual Achievement Review and Evaluation (IAR&E) process, offering coaching and feedback to enhance team performance.
  • Follows organizational policies and procedures, including recruitment and hiring processes.
  • Completes additional tasks as assigned.
  • Responsible for managing medication procurement and documentation in compliance with established protocols.

Physical Demands and Working Conditions:

  • The physical requirements for this position include the ability to handle, feel, and manipulate objects, as well as the capacity to reach, stoop, kneel, and communicate effectively.
  • Regular standing, sitting, and balancing are required, along with the ability to lift and move up to 50 pounds and push wheelchairs weighing up to 200 pounds.
  • Visual acuity for close and distance vision, as well as peripheral vision, is necessary.
  • Effective communication via telephone and regular use of computer screens are essential.
  • The work environment is generally quiet, necessitating good physical and emotional health to fulfill job responsibilities.
