Overview

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing high-quality patient care and coordinating with healthcare professionals to ensure seamless transitions.

Responsibilities

As a Registered Nurse Case Manager, you will:

  • Perform home visits and coordinate patient care with healthcare professionals
  • Oversee the frequency of visits for each patient episode
  • Consult with physicians and office staff to ensure accurate patient care
  • Collect and analyze OASIS data to inform care decisions
  • Ensure medication profiles and lab values are up-to-date
Qualifications

To be successful in this role, you must:

  1. Be a licensed Registered Nurse in the state of employment
  2. Have a minimum of one year of clinical experience
  3. Demonstrate knowledge and skill in current nursing practice
  4. Hold a valid state driver's license and automobile liability insurance
