Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at the Salina, KS Community-Based Outpatient Clinic (CBOC) as a Patient Aligned Care Team (PACT) member. As a PACT RN, you will provide direct care and care coordination to Veterans and their families, working collaboratively with a multidisciplinary team to deliver high-quality, patient-centered care.

Responsibilities
  • Provide direct care and care coordination to Veterans and their families, including health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
  • Manage assigned patient panels, utilize registries, and ensure appropriate evaluation and access is provided to patient panels.
  • Collaborate with the multidisciplinary team to develop, implement, evaluate, and revise policies, procedures, and guidelines.
  • Provide leadership and guidance to peers, supporting the development of a safe and supportive environment conducive to the professional growth and development of healthcare professionals.
  • Evaluate outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • English Language Proficiency.
Preferred Experience
  • RN with clinic experience in the VA, able to multi-task in a busy clinic setting.
Work Schedule

Monday - Friday, 8:00am - 4:30pm. Telework: Not available. Virtual: This is not a virtual position.

Pay Schedule

NZ51.

Requirements
  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
