Join Our Team as a Home Health Registered Nurse

BAYADA Home Health Care is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health Registered Nurse. As a Home Health Registered Nurse, you will have the opportunity to provide high-quality, patient-centered care to individuals in the comfort of their own homes.

About the Role
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address any identified gaps in care.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements
  • Current RN licensure in the state of Pennsylvania.
  • At least one year of recent, verifiable clinical experience in a home health or medical-surgical setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to travel to cases as assigned, with a valid driver's license and reliable transportation.
