We are seeking a skilled Licensed Practical Nurse to join our team at Laurelwood Health Care Center. As a Night Shift LPN, you will be responsible for providing high-quality patient care and ensuring the well-being of our residents.

Key Responsibilities:
  • Direct the daily activities of Certified Nursing Assistants (CNAs) and ensure policies and procedures are followed.
  • Participate in admissions, discharges, and transfers of residents as required.
  • Assess residents' conditions, prepare, and administer medications per physician orders.
  • Initiate, review, and update care plans.
  • Report any changes in a resident's condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
  • Perform other assigned duties given by clinical or administrative leadership.
  • Maintain confidentiality of protected health information, including verbal, written, and electronic communications.
Requirements:
  • Current unrestricted LPN license in practicing state.
  • Current CPR/BLS Certification.
