Job Title: Hospice RN

VNS Health is seeking a skilled Hospice RN to provide personalized nursing and care management to patients in their home or care facilities.

Job Responsibilities:
  • Deliver high-quality patient care in a home or care facility setting
  • Conduct thorough assessments and develop individualized care plans
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Provide education and support to patients and their families
  • Document patient progress and outcomes
Requirements:
  • Current license to practice as a Registered Nurse in New York State
  • Minimum of one year nursing experience in a medical/surgical environment
  • Valid driver's license or NYS Non-Driver photo ID card may be required

Pay Range per Visit: $69.34- $110.00 per visit.
