Key Responsibilities:
- Supervise and actively participate in the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in compliance with current Federal and State regulations.
- Monitor and document the management of Medicare and Managed Care residents in collaboration with facility team members.
- Innovate and implement responsible healthcare solutions through the development of new ideas and processes aimed at achieving exceptional outcomes.
Qualifications:
- Must hold a current, unencumbered, active state license to practice as a Registered Nurse.
- Previous experience in Skilled Nursing or Rehabilitation facilities is preferred.
- A minimum of six (6) months of experience as a MDS Coordinator is required.
- RAC-CT or RNAC certification is preferred.
- Candidates should be qualified, compassionate, and committed to excellence in their work.
