Job Description
">We are seeking a compassionate and skilled Hospice Aide to join our interdisciplinary care team. As a Hospice Aide, you will work under RN supervision to provide quality, continuity, and safe patient care. Your primary responsibilities will include providing patient care, taking vital signs, assisting with self-administered medications, planning nutritious meals, and documenting patient information.
Key Responsibilities

- Provide patient care including bathing, nail/skin care, dressing/undressing, dental hygiene, and other personal hygiene while maintaining a sanitary environment.">
- Offer positive, respectful, and confidential patient/family communication.">
- Taking and recording vital signs, and assisting with self-administered medications.">
- Plan and prepare nutritious meals, including shopping and feeding the patient as needed.">
- Assess and report patient's condition and changes to the RN Case Manager.">
- Assist in ambulation, exercise, transfers, positioning, and supporting complete range of motion and other therapy activities.">

Requirements and Qualifications
">To be considered for this role, you must have:

- Active Certified Nursing Assistant (CNA) designation current with state-specific requirements.">
- Licensed and insured driver with automobile in good working order.">
Estimated Salary: $50,000 - $60,000 per year
