Job Summary

We are seeking a compassionate and committed Social Worker to join our team as a Case Manager in our Community Umbrella Agency. As a Social Worker, you will be responsible for providing a variety of social services to clients and families, including case management, counseling, and referrals to community resources. You will work closely with our team to develop and implement service plans that meet the unique needs of each client and family.

Key Responsibilities
  • Conduct thorough assessments of clients and families to identify their strengths, needs, and goals.
  • Develop and implement individualized service plans that address the unique needs of each client and family.
  • Provide counseling and support to clients and families to help them achieve their goals and improve their overall well-being.
  • Make referrals to community resources and services as needed.
  • Monitor and evaluate the effectiveness of service plans and make adjustments as needed.
  • Collaborate with other professionals, including healthcare providers, educators, and social workers, to ensure comprehensive care and support for clients and families.
  • Participate in case conferences, school conferences, and other meetings as needed to support the movement of cases.
  • Provide on-call services in cases of emergency and on a rotational basis as required by our protocol.
Requirements
  • Bachelor's Degree in Social Work or a related field.
  • At least 2 years of experience in human services, preferably in a case management or counseling role.
  • Valid driver's license with a clean driving record.
  • Ability to obtain and maintain required clearances, including FBI, Child Abuse Clearance, Criminal Background, Driving record, and Medical/TB Test.
