Job Summary

We are seeking a skilled Home Health Aide to join our team. As a Home Health Aide, you will provide personal care services to patients in their homes, ensuring they receive the highest level of care and support.

Responsibilities

  • Provide personal care services to patients, including bathing, dressing, and grooming.
  • Assist with daily living activities, such as feeding, transferring, and toileting.
  • Perform homemaking and other environmental services as assigned.
  • Observe, report, and document any changes in patient status.
  • Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care.
  • Assist patients with prescribed exercises as instructed by nurse or therapist.
  • Perform and record simple urine tests for sugar, acetone, and albumin.

Requirements

  • High school diploma or equivalent required.
  • Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
  • CNA experience within the last two years.
  • Must be able to successfully pass the competency program of the care center.
  • Current CPR certification.
