Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve their overall well-being.
Responsibilities- Conduct thorough adult and geriatric assessments, including medical history, diagnosis, and treatment planning
- Work collaboratively with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document all assessments and care plans accurately and thoroughly, adhering to Matrix Medical Network's guidelines and protocols
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice, with prescription authority or the ability to obtain it
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
