POSITION OVERVIEW:
The Assessment Registered Nurse plays a crucial role in executing UAS-NY evaluations, identifying suitable candidates for membership, and formulating care plans for potential members. This role requires the completion of all productivity metrics and timely submission of necessary documentation. Additionally, the Assessment RN is tasked with articulating managed long-term care benefits and services to prospective members. Continuous communication with the Intake RN Supervisor and UAS reviewers is essential to ensure accurate documentation.
KEY RESPONSIBILITIES:
- Conducts assessments through telehealth, performing comprehensive evaluations (UAS-NY) to ascertain eligibility and initial care plans.
- Creates individualized care plans for prospects based on NYIA assessments.
- Accurately completes the required assessment forms (UAS-NY) and other essential documents in accordance with established policies, procedures, and regulatory guidelines.
- Clearly communicates plan benefits to prospects and referral sources.
- Submits thorough and high-quality documentation promptly.
- Notifies Supervisors of any challenges encountered during assessments.
- Identifies quality of care concerns and escalates issues as necessary, maintaining effective communication with coordinators and supervisors.
- Adheres to all documentation standards set by Centers Plan for Healthy Living.
- Ensures member confidentiality and compliance with HIPAA regulations.
- Engages in regular staff meetings and participates in quality improvement initiatives.
- Performs additional duties as assigned.
EDUCATIONAL BACKGROUND AND EXPERIENCE:
Required Education:
Registered Nursing License
Preferred Experience:
Comprehensive understanding of community-based care programs.
Technical Skills:
Familiarity with Case Trakker, SalesForce, Excel, PowerPoint, and Word is preferred.
Certifications:
Registered Nursing License is mandatory.
Knowledge and Skills:
Demonstrates strong clinical judgment and problem-solving abilities in real-time scenarios.
- Knowledge of MLTCP regulations.
- Ability to collaborate effectively with diverse personalities and work styles.
- Capable of managing multiple tasks simultaneously while meeting deadlines.
- Effective communication skills to educate prospects and take ownership of assigned responsibilities.
PHYSICAL DEMANDS:
- Must be proficient in operating office equipment, including computers and telephones.
- Able to communicate effectively in person and via phone.
