Position Overview

Key Responsibilities:
1. Assess ongoing needs and services of residents to ensure the highest quality of care.
2. Participate in evaluations and documentation for residents experiencing changes in health care services.
3. Conduct comprehensive wellness visits on a monthly basis for all residents.
4. Monitor and follow up with residents who have experienced falls or incidents, ensuring proper documentation and reporting.
5. Maintain open communication with residents' families to address medical needs effectively.
6. Collaborate with attending physicians as necessary to ensure optimal care.
7. Ensure monthly monitoring of weights and vital signs for each resident.
8. Maintain wellness files in accordance with Sunrise policies and regulatory standards.
9. Manage medical supplies and emergency kits within the community.
10. Provide clinical support to team members as required.

Infection Control and Quality Assurance:
1. Adhere to infection control practices and promote standards within the community.
2. Participate in quality assurance meetings and demonstrate knowledge of key quality improvement areas.

Qualifications:
- Graduate of an accredited nursing program.
- Current state license as a Licensed Practical Nurse (LPN).
- Minimum of one year of experience in a healthcare setting such as home health, assisted living, or long-term care.
- Strong assessment skills and ability to manage multiple priorities.
- Excellent written and verbal communication skills.
- Proficient in Microsoft Office and other relevant applications.
- Flexibility to work evenings and weekends as needed.
