- Collaboration and Communication: Work closely with interdisciplinary healthcare teams to develop and implement individualized care plans, ensuring seamless communication and coordination of care.
- Education and Support: Provide education and support to patients and their families on topics such as parenting, substance abuse, stress management, adjustment to illness, trauma, and bereavement, as well as other mental health or emotional difficulties that may impact their care.
- Documentation and Reporting: Maintain accurate and comprehensive records of patient interactions, assessments, and interventions, ensuring timely and effective communication with healthcare teams and stakeholders.
- Professional Development: Stay up-to-date with industry developments and best practices in social work, participating in ongoing education and training to enhance skills and knowledge.
Requirements:
- Education: Master's Degree in Social Work
- Experience: No prior experience required; recent work experience in healthcare preferred
- Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations; strong analytical and problem-solving skills; ability to maintain organization in a fast-paced environment
- Licensure/Certification: License in Social Work preferred; eligible to sit for and pass the test of certification as a Certified Case Manager (CCM) or Accredited Care Manager (ACM) preferred
